An evidence ledger tracks each claim that moves from a CIM into the memo. One row per claim: source document and page, extracted value, transformation, verification status, reviewer and open issues.
Unsupported claims are dropped or flagged, never softened. The ledger gives the QoE provider a question list instead of a blank page.
